Use this form if you would like to register for the Camporee.

PLEASE NOTE: You will not be registered until the registration fee is received. The fee ($85.00) per Cadet or Counselor should be sent to: Mr. Robert Neutel R. R. # 6 Woodstock, Ontario N4S 7W1 Make cheques payable to "2001 All Ontario Camporee" Please send a seperate form for each registrant:

All registrations and fees must be in and paid by May 15, 2001 in order to be ensured of a place at the Camporee.

To be eligable each registrant (Cadet or Counselor) must have completed the NEW Camporee Certification Course

Cadets must be TEN(10) years of age by Dec 31, 2001

PARENTS (GUARDIAN) CONSENT FORM:

I (We) do hereby give permission for my (our) son to attend the ALL ONTARIO CAMPOREE from July 23 - 27,2001. I (We) understand that no fire crackers, fireworks or sirnilar explosive material may be taken to the Camporee, Further, I (We) understand that should my (our) son be found in possession of same, OR if he is acting in a manner that the camp director deems it necessary to expel him for the remainder of the Camporee, I (We) will be responsible for transporting my (our) son home.

I (We) also hereby give permission for the Camp Staff and/or Doctor in charge, to authorize any Emergency Medical or Surgical Attention they believe to be in my son's best interest.
